1948 年 Framingham 心臟研究對「猝死」(sudden death)下的定義,至今仍是醫學研究中最常被採用的定義。根據 Framingham 的定義,「猝死」是指徵狀出現後一小時內的死亡(Best,n.d.)。
雖然經常做運動能降低許多慢性疾病(包括心血管疾病)的死亡風險(U.S. Departement of Health and Human Services,1996),但心血管疾病患者在運動時出現心血管問題的機會卻比健康正常的人要高出 10 倍(Fletcher 等,1995)。Balady 等(1998)指出,對於一向缺乏運動的人來說,中等劇烈的體力活動有可能觸發心肌缺血的情況(ischemic cardiac events)。Siscovick 等(1984)曾考查 25 至 75 歲男性出現原發性心搏停止(primary cardiac arrest)的個案,他們發現劇烈運動雖然會增加原發性心搏停止的風險,但對於一個健康正常的社群來說,出現這情況的機會為每年 0.55 宗 / 10,000 名男性。
除了運動時死亡個案的研究外,Framingham 心臟研究的結果顯示,一個 50 歲,沒有患上糖尿病的非吸煙者,出現心肌梗塞(myocardial infarction)的機會約為每年 1% 或每小時 1/1,000,000(Anderson 等,1991),但 4.4 至 7.1% 的心臟病患者在劇烈運動(>= 6 METs)開始後 1 小時內便出現急性心肌梗塞的情況(Mittleman 等,1993;Willich 等,1993)。那些沒有定期進行體力活動的人,其相對風險也較高,而且早上出現事故的機會更是其他時段的 3 倍。Tofler 等(1992)發現在 3,339 名心臟病患者當中,18.7% 在出現心肌梗塞前曾進行中等強度或以上的體力活動。
至於運動員方面,猝死的情況其實並不常見,以美國為例,每年有 10 至 13 宗個案(Maron 等,1996;Van Camp 等,1995),與其他死亡個案相比,美國每年有 14,700 個年青人死於交通意外、400 人死於火災和 250 人死於吸入有毒氣體(National Safety Council,1985)。至於運動項目方面,則以籃球和美式足球運動員的風險較高(Maron 等,1996),而大部分的猝死個案都是發生於練習或比賽進行中或之後的一段短時間之內(Thomas,1999)。
由於運動員(特別是著名的運動員)是大眾的焦點人物,給人的印象也是最健康強壯的一群,所以運動員猝死的新聞也就特別觸目。運動中猝死的個案每多涉及當事人未有察覺自己患有嚴重的心血管疾病,但這類個案出現的機會在運動員的社群中仍然並不普遍,根據 Noakes(1988),約為 1 / 10,000(活躍運動人士)至 1 / 200,000(兒童及年青人)。
Maron 等(1996)及 Van Camp 等(1995)就曾做過兩項大型研究,考查年青競賽運動員猝死(sudden death)的原因,他們發現當中最常見的就是心肌肥大病(hypertrophic cardiomyopathy)及冠狀動脈異常(coronary artery anomalies);他們研究結果的比較見表一(Wen,2004)。至於 35 歲以上的死者,Maron 等(1996)發現約 80% 是由於冠心病致死。

| 整體來說,一般人(特別是健康正常的人)於運動時死亡的風險並不高,每年約有 0.75 / 100,000 年青男性運動員和 0.13 / 100,000 年青女運動員在運動時死亡(Van Camp 等,1995),而中年男性則為 6 / 100,000(Thomson,1996);中年女性及老年人則未有明確的統計數字(Balady 等,1998)。突然死亡的運動員都患有嚴重的心臟毛病,所以無論他們是否做運動,突然死亡的風險都是很高。為安全計,在開始任何運動訓練計劃前,最好都先作醫學檢測。
